Our Mission is to provide professional and paraprofessional services to our clients in their homes, and to assist them in achieving their highest level of potential in their day-to-day self-care activities.

OUR STORY

AM Home Health Care (AMHHC) is a Home Health Care & Personal Care Agency that was founded by in 2012 by Zakawan Din. This agency consists of Bilingual employees from diverse background, with the ability to speak one or more of the following languages: English, Burmese, Karen, etc.


This agency was founded solely for the purpose of giving you or your loved one the care they need in their own home, where they feel safe and comfortable.


Our mission is to provide professional and paraprofessional services to our client in their homes, and to assist them in achieving their highest level of potential in their day-to-day self-care activitied.


We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional's point of view.

We know it's difficult to invite a stranger into your home or the home of your loved one to received care. So, we take great care in selecting the caregivers we choose to work with our clients. 


All employees of AM Home Health Care must meet the high standards of our agency policies. Not only were they licensed or certified by the State of Indiana but also completed and passed Criminal Background Check, Reference Check and Hands-on Skilled Check supervised by our Registered Nurse.

Sometimes, we need care only a trained medical professional can provide. Skilled Nursing Care can provide all those services, meeting a specific set of criteria that must be performed by a Registered Nurse (RN), Licensed Practical Nurse (LPN), or a certified Home Health Aide (HHA).

Our caregivers assist with activities of daily living, including bathing, dressing, grooming, housekeeping, meal planning and preparation. Helping individuals maintain independence and dignity. As well as maintaining nutritious and delicious meals.

AM Home Health Care is a locally owned company which is continuing to grow, and we are looking for caring individuals who are willing to learn and care for our clients. All our clients are in the Fort Wayne area. 


Job Type: Full-Time and Part-Time


Benefits: 

  • Bi-Weekly paychecks 
  • Flexible Hours 
  • Health insurance 
  • Starting wage of up to $16:00 - $22:00 (Depends on your home care experience) 
  • Overtimes are multiple by 1 1/2-time base on your pay rate
  • Time off benefits: Full-Time & Part-Time Employees
  • PTO - paid 2 weeks a year
  • Holiday - paid 6 days a year’s
  • Employee referral program - for referrals who get hired and stay through their 180 days probationary period or 1000 hours. ($500.00)  
  • Awards & Recognition programs 


Requirements: 

  • Must have a valid driver's license and reliable vehicle
  • Open availability every other weekend
  • Ability to lift 50 pounds
